Equivalent doses of Oestrogen replacement when using HRT during the perimenopause and menopause

**It should be noted that response to any preparation is unique to each woman, some women responding well to a low dose of one preparation while not responding well to a high dose of another.

Product Ultra low Low Medium High
Oral HRT tablets 0.5mg 1.0mg 2.0mg 3-4mg
Patch Half 25 25 50 75-100
Gel - pump 1/2 pump 1 pump 2 pumps 3-4 pumps
Gel - sachet 1/2 x 0.5mg sachet = 0.25mg 0.5mg 1-1.5mg 2-3mg
Spray 1 spray 2 sprays 3 sprays (see below)
  • Each Lenzetto spray delivers 1.53mg of oestradiol, but only a portion of that is absorbed into the bloodstream. The absorption rate is lower and more variable than gels or patches. Because of this, the maximum licensed dose is three sprays per day, which is roughly equivalent to a 50mcg patch or two pumps of Oestrogel. Higher doses aren’t currently licensed with Lenzetto, even though some women may need more oestrogen to manage symptoms. This is partly due to how the spray interacts with skin and how consistently it’s absorbed - which can vary based on skin thickness, hydration, and application technique.

*Not all suggested doses are within Summary of Product Characteristics, so if used, are used off licence.

**If you still have your womb, you’ll also need a progestogen as part of your HRT to protect the lining of the uterus.
