Equivalent doses of Oestrogen replacement in your HRT

Published on 30 October 2025 at 15:00

Approximate equivalent doses of Oestrogen replacement for those of average menopause age with symptoms (45-55 years)

**It should be noted that response to any preparation is unique to each woman, some women responding well to a low dose of one preparation while not responding well to a high dose of another.

(The below chart was updated according to the British Menopause Society guidelines  in June 2026)

Product Low Medium High
Oral HRT tablets 0.5mg 1-2mg 3.0mg
Patch 25/30/37.5 mcg 40/50/75mcg 100mcg
Gel - pump 1 pump 2-3 pumps 4 pumps
Gel - sachet 0.5mg 1-2mg* 2.5-3mg *
Spray 1-2 sprays 3 sprays - 4 sprays* 5-6 sprays**

* Outside of Summary of Product Characteristic licensing

  • Each Lenzetto spray delivers 1.53mg of oestradiol, but only a portion of that is absorbed into the bloodstream. The absorption rate is lower and more variable than gels or patches. Because of this, the maximum licensed dose is three sprays per day, which is roughly equivalent to a 50mcg patch or two pumps of Oestrogel. Higher doses aren’t currently licensed with Lenzetto, even though some women may need more oestrogen to manage symptoms. This is partly due to how the spray interacts with skin and how consistently it’s absorbed - which can vary based on skin thickness, hydration, and application technique.

  • **If using more than 3 sprays apply these additional sprays to the opposite inner forearm.

If you still have your womb, you’ll also need a progestogen as part of your HRT to protect the lining of the uterus.

Please help as I am very confused. I have been on 4 pumps of oestrogel daily due to some osteoporosis in my lower spine and my menopause doctor in the Uk was very happy with my oestrogen levels. I moved to Mallorca earlier in the year and Oestrogel is no longer available to me. I have found a doctor who wants me to switch to Lenzetto. She has told me that one pump of Lenzetto is equivalent to 2 pumps of oestrogel. In view of the fact that I have some osteoporosis it is critical to me to keep my oestrogen levels at the optimal level and I don't think she is correct in her interpretation of the doses. Would you be kind enough to let me know what dose of Lenzetto I should be on if I am currently on 4 pumps of Oestrogel? With many thanks, Alison

I’m on lenzetto as patches come off my skin.
The last ones I was allergic too.
I have PMDD for 10 to 14 days and lenzetto has increased my bleeding I was ok on original estrodial clear patches but they kept peeling off,
Had to go private for patches and lenzetto plus fortnights pills and internal stuff for dryness.
I still have my womb
Lenzetto doesnt help my PMDD at all.
